CQC: Risks not adequately mitigated during Calocane's care

Care Quality Commission's Chris Dzikiti has said medical and police records showed Valdo Calocane would have relapsed into "distressing and potentially aggressive" behaviour.

Dzikiti, who is the lead author in the report into Calocane's history, added the risks were not adequately mitigated throughout the course of his mental health care.
